As a leading public player in digital health in Guadeloupe, GRADeS Archipel 97-1 works to improve the coordination of care pathways across the region. Faced with the growing complexity of care and the multitude of stakeholders involved, GRADeS has chosen to rely on Odycare, in collaboration with Coexya.
A specialist in digital health for over 20 years, Coexya currently supports eight French regions in the implementation of healthcare coordination projects, including Guadeloupe, drawing on a proven software solution and in-depth expertise in regional issues.
The first users of Odycare in Guadeloupe: DAC and School Obesity Program
In Guadeloupe, the first users of Odycare are the Coordination Support Units (DAC). These structures provide support to health, social, and medico-social professionals who work with people facing multiple challenges and complex health needs.
Thanks to Odycare, Guadeloupe’s two DACs now have a set of tools that facilitate the monitoring and coordination of complex care pathways: shared patient records, instant messaging, monitoring forms, notifications, and information sharing between the various stakeholders. The solution thus contributes to better continuity of care and enhanced coordination around the patient.
Odycare is also used in middle schools as part of the PRALIMAP-INES program.
This program aims to support overweight or obese adolescents by offering tailored, coordinated support based on screening carried out in schools. This support consists of five group sessions with a multidisciplinary team of dieticians, psychologists, and physical activity professionals.
